Childhood adversity is one of the strongest predictors of adolescent mental illness. Therefore, it is critical that the mechanisms that aid resilient functioning in individuals exposed to childhood adversity are better understood. Here, we examined whether resilient functioning was related to structural brain network topology. We quantified resilient functioning at the individual level as psychosocial functioning adjusted for the severity of childhood adversity in a large sample of adolescents (N = 2406, aged 14–24). Next, we examined nodal degree (the number of connections that brain regions have in a network) using brain-wide cortical thickness measures in a representative subset (N = 275) using a sliding window approach. We found that higher resilient functioning was associated with lower nodal degree of multiple regions including the dorsolateral prefrontal cortex, the medial prefrontal cortex, and the posterior superior temporal sulcus (z > 1.645). During adolescence, decreases in nodal degree are thought to reflect a normative developmental process that is part of the extensive remodeling of structural brain network topology. Prior findings in this sample showed that decreased nodal degree was associated with age, as such our findings of negative associations between nodal degree and resilient functioning may therefore potentially resemble a more mature structural network configuration in individuals with higher resilient functioning.

Objective:
To evaluate the impact of a diagnostic stewardship intervention on Clostridioides difficile healthcare-associated infections (HAI).
Design:Quality improvement study.
Setting:Two urban acute care hospitals.
Interventions:All inpatient stool testing for C. difficile required review and approval prior to specimen processing in the laboratory. An infection preventionist reviewed all orders daily through chart review and conversations with nursing; orders meeting clinical criteria for testing were approved, orders not meeting clinical criteria were discussed with the ordering provider. The proportion of completed tests meeting clinical criteria for testing and the primary outcome of C. difficile HAI were compared before and after the intervention.
Results:The frequency of completed C. difficile orders not meeting criteria was lower [146 (7.5%) of 1,958] in the intervention period (January 10, 2022–October 14, 2022) than in the sampled 3-month preintervention period [26 (21.0%) of 124; P < .001]. C. difficile HAI rates were 8.80 per 10,000 patient days prior to the intervention (March 1, 2021–January 9, 2022) and 7.69 per 10,000 patient days during the intervention period (incidence rate ratio, 0.87; 95% confidence interval, 0.73–1.05; P = .13).
Conclusions:A stringent order-approval process reduced clinically nonindicated testing for C. difficile but did not significantly decrease HAIs.

Exclusion of special populations (older adults; pregnant women, children, and adolescents; individuals of lower socioeconomic status and/or who live in rural communities; people from racial and ethnic minority groups; individuals from sexual or gender minority groups; and individuals with disabilities) in research is a pervasive problem, despite efforts and policy changes by the National Institutes of Health and other organizations. These populations are adversely impacted by social determinants of health (SDOH) that reduce access and ability to participate in biomedical research. In March 2020, the Northwestern University Clinical and Translational Sciences Institute hosted the “Lifespan and Life Course Research: integrating strategies” “Un-Meeting” to discuss barriers and solutions to underrepresentation of special populations in biomedical research. The COVID-19 pandemic highlighted how exclusion of representative populations in research can increase health inequities. We applied findings of this meeting to perform a literature review of barriers and solutions to recruitment and retention of representative populations in research and to discuss how findings are important to research conducted during the ongoing COVID-19 pandemic. We highlight the role of SDOH, review barriers and solutions to underrepresentation, and discuss the importance of a structural competency framework to improve research participation and retention among special populations.

The aim of this study was to examine cross-sectionally whether higher cardiorespiratory fitness (CRF) might favorably modify amyloid-β (Aβ)-related decrements in cognition in a cohort of late-middle-aged adults at risk for Alzheimer’s disease (AD). Sixty-nine enrollees in the Wisconsin Registry for Alzheimer’s Prevention participated in this study. They completed a comprehensive neuropsychological exam, underwent 11C Pittsburgh Compound B (PiB)-PET imaging, and performed a graded treadmill exercise test to volitional exhaustion. Peak oxygen consumption (VO2peak) during the exercise test was used as the index of CRF. Forty-five participants also underwent lumbar puncture for collection of cerebrospinal fluid (CSF) samples, from which Aβ42 was immunoassayed. Covariate-adjusted regression analyses were used to test whether the association between Aβ and cognition was modified by CRF. There were significant VO2peak*PiB-PET interactions for Immediate Memory (p=.041) and Verbal Learning & Memory (p=.025). There were also significant VO2peak*CSF Aβ42 interactions for Immediate Memory (p<.001) and Verbal Learning & Memory (p<.001). Specifically, in the context of high Aβ burden, that is, increased PiB-PET binding or reduced CSF Aβ42, individuals with higher CRF exhibited significantly better cognition compared with individuals with lower CRF. In a late-middle-aged, at-risk cohort, higher CRF is associated with a diminution of Aβ-related effects on cognition. These findings suggest that exercise might play an important role in the prevention of AD. (JINS, 2015, 21, 841–850)

Summary
In 1371, Nicholas Oresme, scholar, academic, councillor of Charles V and later Bishop of Lisieux, completed a translation and commentary in French on the Pseudo-Aristotelian Livre Yconomique, and one on Aristotle's Politics, companion works to his earlier commentary on the Ethics. Produced under the aegis of a publication program of Charles V, Oresme's commentaries are remarkable for many reasons. Both text and gloss are in French, in a striking departure from the usual scholastic context of such works, and they thus introduce many neologisms and new terms for concepts. They are found in deluxe manuscripts, with wonderful programs of illustration, as befitting royal commissions. Oresme's glosses are designed for a different reader than academic commentaries, because they aim at forming an educative program in the public good. But the best reason for calling attention to them is for their originality and charm. For example, in speaking of the proper deportment of married couples, Oresme's gloss surprises and delights by its etymological literalism (where ‘T’ is text and ‘G’ is gloss):
[T. For the outward adornment of the couple may be no different from the speeches commonly associated with tragedies.
G. That is to say that such an excess of affectation is ugly and unseemly, like the words used in tragedies. Or according to another commentator, such a thing gives opportunity for evil tongues, such as one hears in tragedy. Tragedies are ditties and rimes about base and improper things. And the word is derived from tragos in Greek, meaning goat or stinking animal, because to symbolize the filthy words and scandal of these rimes, a goat was sacrificed.]

A simultaneous investigation into certain epidemiological aspects of scarlet fever was carried out over a 2-year period at Oxford, Cambridge and Cardiff. The results obtained at the three centres are compared and contrasted.
Nose and throat swabs were taken from 648 patients on admission to hospital; group A haemolytic streptococci were demonstrated in the swabs of 75·3 %. Eight days after admission 81·9 % had given positive swabs.
Serological typing showed that 56·9 % of strains were types 1–4. The only other common types were types 11 and 8/25.
The average length of stay in hospital was estimated for all cases, divided into groups according to whether or not they had suffered from complications or been cross-infected. The average for the whole series was 30·6 days; this was increased to 33·4 days for all complicated cases and to 37·6 days for all those cross-infected. It was longest (42·3 days) when there were both complications and cross-infections.
The incidence of cross-infection with fresh serological types of haemolytic streptococci was noted both in hospital and for the first 3 weeks after discharge. In the whole series 20·1 % of cases were cross-infected in hospital. The cross-infection rate was highest at Cardiff (27·9 %) and lowest at Oxford (13·5 %). Of the 123 cross-infected cases, complications occurred in 23·6%.
A total of ninety-eight patients (16·4 %) suffered from complications; in twenty-three of these (23·4%) complications were attributed to cross-infecting strains.
Nose and throat swabs were taken from all patients shortly before discharge; 60·3% were still carrying haemolytic streptococci.
Data were obtained regarding the rate at which cases became free of haemolytic streptococci. By the 10th week of the disease only 3·2 % of those who had not been cross-infected were carrying streptococci. Cross-infected patients were slower in becoming negative; at the 10th week 25·8% were still carriers.
There were fourteen return cases (return case rate 2·1%) following the return home of eleven primary cases (infecting case rate 1·7 %).
Swabbing of home contacts at the time of the patient's admission to hospital showed that one or more contacts of 25·5% of cases carried in their naso-pharynx the same type of haemolytic streptococci as that infecting the patient; 12·8% of the contacts swabbed were positive. During the first 3 weeks after discharge 24·6 % of contacts, that is one or more contacts of 55·5% of cases, carried a type that had been harboured by the patient while in hospital.
It was noted that in a particular area the common types that gave rise to scarlet fever also commonly caused other streptococcal infections of the upper respiratory tract. Type 12, however, though frequently found in other streptococcal infections and in healthy carriers, appeared only rarely to cause scarlet fever.
It is suggested that the low incidence of complications, less intimate relationship between complications and cross-infections and low return case rate found in this investigation, as compared to earlier observations made in England, are associated with lowered virulence and invasive powers of the strains of Str. pyogenes responsible for the much milder scarlet fever now prevalent.
The bacteriological findings confirm the wisdom of modern fever hospital practice of early discharge from hospital. It is suggested that discharge within 3 weeks should be the universally adopted rule.

A vitrification technology utilizing a lanthanide borosilicate (LaBS) glass is a viable option for dispositioning excess weapons-useable plutonium that is not suitable for processing into mixed oxide (MOX) fuel. A significant effort to develop a glass formulation and vitrification process to immobilize plutonium was completed in the mid-1990s. The LaBS glass formulation was found to be capable of immobilizing in excess of 10 wt % Pu and to be tolerant of a range of impurities. A more detailed study is now needed to quantify the ability of the glass to accommodate the anticipated impurities associated with the Pu feeds now slated for disposition.
The database of Pu feeds was reviewed to identify impurity species and concentration ranges for these impurities. Based on this review, a statistically designed test matrix of glass compositions was developed to evaluate the ability of the LaBS glass to accommodate the impurities. Sixty surrogate LaBS glass compositions were prepared in accordance with the statistically designed test matrix. The heterogeneity (e.g. degree of crystallinity) and durability (as measured by the Product Consistency Test – Method A (PCT–A)) of the glasses were used to assess the effects of impurities on glass quality.

Vitamin E is the major lipid-soluble antioxidant in human subjects and is crucial in protecting polyunsaturated fatty acids (PUFA) against lipid peroxidation. Dietary PUFA have been suggested to inhibit the absorption of vitamin E. The present study in young male rats was designed to investigate the effect of increasing concentrations of dietary linoleic acid on the faecal excretion of vitamin E. The rats were fed on semi-synthetic diets containing two concentrations of fat (59 g/kg diet, 15 energy % (en%) or 131 g/kg, 30 en%) for 3 weeks. Triacylglycerol rich in linoleic acid was added at the expense of triacylglycerol rich in saturated fatty acids to obtain dietary concentrations of 13,39 or 66 g linoleic acid/kg diet for the high-fat diet (131 g fat/kg) and 12, 24 or 36 g linoleic acid/kg diet for the reduced-fat diet (59 g fat/kg). The results from the present study demonstrate that the faecal excretion of vitamin E was significantly lower in rats fed on diets with high levels of linoleic acid compared with rats fed on lower levels of linoleic acid irrespective of the dietary fat content. The concentration of vitamin E in liver and plasma was significantly lower in animals fed on the highest concentration of linoleic acid compared with those fed on the lowest level. Results from the present study also demonstrate that at the same concentration of linoleic acid, the faecal excretion of vitamin E in rats fed on reduced-fat diets was significantly lower than in rats fed on high-fat diets. Our findings indicate that the apparent absorption of vitamin E is not inhibited by dietary PUFA. Results from the present study also demonstrate that a reduction of dietary fat content from 30 en% to 15 en% does not lower the apparent absorption of vitamin E
